This form is for members to request approval from the Management Committee to request an item costing more than $100 or to request some work be done. It can also be used to make a suggestion for the development of Shed facilities or equipment. The form collects the basic information that the Management Committee needs to consider proposals. Attach any supporting documents, such as more detail, quotes etc. that won’t fit on the form.
Leave the completed forn in the Secretary tray in the lunch room.
